Breaking changes

• Require node >= v16 v18
• Make --root the default in workspaces mode #1353

Risky changes

• Replace pacote with npm-registry-fetch #1329
  • This should not be breaking, but risk is deemed to be high given that is involves swapping out the core fetch lib.
